The SNAP-IV 18-item scale is an abbreviated version of the SNAP Questionnaire to screen for attention-deficit/hyperactivity disorder (ADHD). There are 2 parts of this test covering 2 subsets of symptoms: Inattention (items 1-9) and Hyperactivity/Impulsivity (items 10-18). This is Part 2, please make sure to complete part 1 of this test too.
